1. Download and print the forms in this packet.
2. Fill out the forms and indicate which tests you want.
3. Pack up a plastic bag of water, and double bag it.
4. Pack up three sick, affected fish with a Bag Buddy under pure oxygen and double bag them, put them on an ice pack and ship them with your water sample.
5. Follow packing instructions and shipping instructions.
6. Ship overnight to appropriate address.

! - Payment must be included in the shipment or the samples will be left on the doorstep until your payment details are sent or collected.
! - All paperwork sent with fish should be packed dry inside a sealed Ziploc baggie and if possible, not placed in or on the ice in the box. Formwork must arrive legible and dry!
! - If your swabs for KHV are received submerged in water, mixed in ice, they will be discarded.
What if I want different-fish tested for different-things?
You will have to pack the fish individually in the boxes. Catch several sick but alive fish if possible. You may want a fish to come from each of your tanks to identify which tanks have what. Take pictures of the fish and print the pictures, or do the best possible job of describing the fish, or simply mark their respective bags with a permanent marker. Identify the fish A, B and C. Pack the fish in double fish-bags under pure oxygen and double or triple band the bag tops. Use ice or ice packs in the boxes to keep the fish cooler. Jungle Bag Buddies also help considerably, one tablet per gallon of shipping water.
Pack with the fish, in separate, dry Ziploc bags:
One completed copy of the appropriate testing-request form.
Ship live specimens via Airborne Express, UPS, FedEx, or other competent carrier with completed forms overnight to UGA for morning delivery.
WARNING: If a fish specimen has been dead for more than a few (4) hours and the lenses in the eyes are white, the fish has a fetid rotting smell, or the internal organs have turned into a foul smelling emulsion, there will be no viable results.
Fish that are alive, shipped alive, or which are recently deceased and packed on ice are the best samples to submit.
